All memberships include a one-time sign-up fee of $98 to establish care. Children's sign-up fee is reduced to $50 when paired with a paying adult member.

Whether you need a one-time visit or occasional check-ins, you'll receive thoughtful, focused attention at every appointment. No commitment required, just great care when you need it.
New Patient Visit: $250
Your first step to care at Rooted Health. This initial visit
(in-person or telehealth) establishes your health history and gives us a comprehensive picture of your needs. Valid for 7 days from purchase.
In-Person Follow-Up: $175
For in-person follow-up appointments or complex reviews. Valid for 7 days from purchase.
Standard Telemedicine Follow-Up: $150
A convenient telehealth follow-up for ongoing concerns. Requires a completed New Patient Visit first. Valid for 7 days from purchase.
